Incident Investigation
EXOS 4579 is offered this winter:
This seminar will step you through the investigation of “unplanned events.” Learn the critical parts of the initial response, securing the area, gathering evidence, interviewing techniques, documentation, role of various stakeholders, reporting formats, etc. Also learn the primary reasons for performing incident investigations.

By the end of this course, you should be able to:
- Recognize the importance of the accident theory within incident investigation practice.
- Describe the processes involved in determining the costs of incidents.
- Identify best practices in preserving fragile information during incident investigations.
- Describe key strategies for collecting pertinent information for analysis following an incident.
- Analyze a case to establish a probable cause and a potential contributing factor.
- Identify communication strategies to effect change and prevent recurrence.
- Prepare a follow-up plan to ensure post-incident safety recommendations are implemented.

Timothy Reteff (B.A., CRSP) has worked in health and safety for more than a decade. He started his career as a hazmat tech in the consulting industry, where he oversaw the abatement and remediation of hazardous materials from sites ranging from residential to commercial to industrial. Working with Alberta Occupational Health & Safety since 2012, Tim has been involved in a number of fatality investigations and countless reportable incident investigations. It is his belief that safety is no accident and that almost every incident is preventable with the correct amount of due diligence and proper planning.
